These sheets contain information regarding major requirements and allow you to track your progress. Make sure you are following the requirements for the year in which you declared the major.


You are evaluated for graduation using the information on your progress report. Thus, it is important to know how to read it. Progress reports can be accessed through Route Y.



If you have already applied for graduation and need to make any changes (ie: change graduation date, add or drop minors, etc.), you will use this form to do so.
Students who wish to walk in a convocation ceremony other than the one in which they are graduating can use this form to make that request. We are now accepting request for the April 2008 ceremony.

Based on the Family Educational Rights and Privacy Act (FERPA) which governs access to student records and information, BYU is required by law to not allow access to a third party unless we have written permission from the student. This form may be used to grant permission to the advisement center to discuss your record with a spouse, family member, or friend.
